Program & Services
FAU Community Health Center
Florida Atlantic University/Northwest Community Health Alliance provides various services directed by advanced practice nurses in collaboration with other healthcare providers to provide person-centered services. FAUCHC is certified by the American Diabetes Association to provide diabetes self-management training to patients, families, and community members. FAUCHC values establishing an ongoing authentic relationship with Center for Child Counseling, Children Home Society, Palm Beach Atlantic University, and our community partners.
The Community Outreach Team travels throughout Palm Beach County to provide services where people work and live. We provide early identification of physical, mental, and chronic illnesses through health care screenings. Our dedicated professionals work with several community partners to build strong and healthy communities. This allows us to serve as your liaison between various organizations by removing barriers and enhancing support in healthcare and with safety concerns.
Bridges Implemented By CHS
Bridges, implemented by the Children’s Home Society, is a community-based program designed to support underserved mothers and their families. The program provides parenting workshops, practical tips for raising young children, and guidance on fostering healthy child development. Additionally, Bridges connects families with essential community resources to address their needs, helping them build a strong foundation for growth and learning.
